The Role We are seeking a compassionate and skilled Licensed Clinical Social Worker/Medical Social Worker to join reputed company. This position involves providing comprehensive psychosocial assessments, counseling services, and care coordination for patients and their families reputed company a reputed company setting. The ideal candidate will have experience in reputed company environments and a strong commitment to patient-centered care. Experience in Cardiology preferred. An Arizona license and availability to work standard business hours (9 AM - 5 PM) in Pacific Time are required.
Responsibilities
Clinical Services Conduct comprehensive psychosocial assessments to identify patient and family needs reputed company and implement treatment plans in collaboration with interdisciplinary teams Conduct discharge planning and coordinate community resources Care Coordination Collaborate with physicians, nurses, and other reputed company to ensure holistic patient care Coordinate referrals to community resources, support services, and specialty care Facilitate communication between patients, families, and medical teams Participate in interdisciplinary team meetings and case conferences Patient and Family Support Provide emotional support and counseling to patients facing medical diagnoses reputed company patients and families about medical conditions, treatment options, and coping strategies Assist with advance reputed company planning and end-of-life care discussions Documentation and Compliance Maintain accurate and timely clinical documentation in proprietary technology and/or electronic health records Ensure compliance with HIPAA, state regulations, and hospital policies Complete required assessments reputed company established timeframes
Qualifications
Education and Licensure Master's degree in Social Work (MSW) from a CSWE-accredited program reputed company Arizona state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent Valid state professional license in good standing Experience Minimum two years of clinical social work experience preferred Experience in reputed company/medical settings strongly preferred Knowledge of medical terminology and reputed company systems Experience with electronic health record systems
