Summary The Population Care Nurse is a registered nurse (RN) who focuses on managing and improving the health outcomes, wellness and quality performance of specific populations or communications, particularly those with chronic conditions, high reputed company needs, or underserved groups. Job Description

The nurse implements provider-based interventions to improve health, wellness and quality performance. Education:
- Bachelors Degree in Nursing REQUIRED
Certification/Licensure
- Registered Nurse (RN) License (Compact or Virginia) REQUIRED
Experience:
- 2 years Nursing experience REQUIRED
- Health Plan experience preferred
