Key Responsibilities

• Conduct comprehensive assessments of members' clinical needs and develop personalized care plans
• Collaborate with healthcare providers to ensure coordinated care and optimal health outcomes
• Utilize clinical judgment to identify and address potential health risks and barriers to care
• Develop and implement strategies to promote member engagement and empowerment
• Participate in case conferences and multidisciplinary team meetings to ensure comprehensive care planning

Requirements

• Active and unrestricted RN licensure in the state of California
• 5+ years of clinical experience
• Bachelor's degree in Nursing or related field
• Case management experience in an integrated model (preferred)
