About the position Sentara Health is hiring for an Integrated Care Manager/RN in Tidewater, VA! This is a full-time, permanent position requiring 40 hours of work per week, with standard working hours from 8 am to 5 pm EST, Monday through Friday. The role is primarily remote but will involve conducting home visits for members of the Long-Term Services and Supports (LTSS) population. Candidates must reside in the Tidewater area, specifically in Norfolk, Virginia Beach, Suffolk, Portsmouth, or Chesapeake, VA. As an Integrated Care Manager with Sentara, you will work collaboratively with patients, their families, and members of the medical team to provide essential resources and support to prevent hospital readmissions. Your primary responsibility will be to customize care plans that best meet the needs of patients, which may include coordinating home health services, long-term care placements, home medical equipment, and connecting patients with community resources. The teamwork within the organization is exceptional, with a strong focus on ensuring the best possible care for patients. This position requires a minimum of three years of nursing experience and a valid Registered Nurse (RN) license in Virginia or a multi-state/compact license. For those in Integrated Care Management departments, obtaining specialty certification (such as ACM, CCM, CCCTM, or RN-BC) is required within one year of eligibility. Preferred qualifications include three years of case management experience.
