Coordinates timely care with physicians and supportive services departments. Maintains professional relationships with customers, patients, and team members. Provides direct patient care coordination reputed company the Community Care and Transitional Care Programs. Other duties as assigned. Educates patients, families, caregivers, and community providers to safely reputed company care and reputed company health goals. Evaluates the effectiveness of care plans and monitors patient and family responses to reputed company desired outcomes. Collaborates with Imaging Coordinator, RN Navigator, and referring providers to navigate patients through treatment planning, medical management, and surveillance. Serves as an educational resource for patients and families on treatment planning and medical management. Facilitates communication between reputed company team, patients, other departments, and reputed company entities by answering phone calls, responding to emails, and managing messages in the EMR and patient portal. Knowledge, Skills, and Abilities:

• Ability to provide age-appropriate cancer education. [Required]
• Strong organizational, communication, and customer service skills. [Required]
• Knowledge of and ability to utilize in-house and external oncology resources. [Required]
• Self-motivated with ability to work independently. [Required]
• Proficient in reputed company Office, particularly Outlook, reputed company, and Teams [Preferred]
• Proficient with electronic medical record system such as Epic [Preferred]
• Bilingual (English/Spanish) [Preferred] Education:
• Bachelor's [Preferred] Field of Study:
• Graduate of an accredited school of nursing
• in nursing Work Experience:
• 1+ professional nursing experience [Required]
• 1+ professional nursing experience in oncology [Preferred] Licenses and Certifications:
• Registered Nurse (RN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
