Job Description Description: The RN or LPN Admissions Nurse is responsible for facilitating the admission process for new residents, ensuring their smooth transition into our facility, and providing high-quality nursing care. This position requires strong clinical skills, excellent communication abilities, and a commitment to delivering quality care to our residents. Applicants must be friendly, positive, detail-oriented, can adapt well to changing and diverse responsibilities, and must have the skills to resolve concerns from patients, patient family members, and staff. We are a 16-bed short-term only rehabilitation facility. Most of our patients stay for 2-3 weeks before they are recovered and return home. We are a newer facility looking for someone that can help us create successful patient outcomes and maintain our 5-STAR reputed company. Schedule: PRN, On-call Weekends Primary Responsibilities and Duties: Conduct thorough assessments of incoming residents to gather medical history, assess reputed company health status, and identify care needs. Collaborate with physicians, nursing staff, and other members of the team to reputed company individualized care plans for new residents. Communicate effectively with residents, their families, and caregivers to provide information about the admissions process, facility policies, and available services. Coordinate with other departments, such as dietary and therapy, to arrange for necessary assessments and services for new admissions. Administer medications and treatments as prescribed by the physician, following established medication administration protocols. Requirements: Job Qualifications: Valid Registered Nurse License (RN) in the state where work is performed. 1 or more years of experience as an admission nurse or intake coordinator in a reputed company setting in a skilled nursing facility or long-term care environment preferred.
