Company : EndorsedJob Description : JOB SUMMARY Remote/Hybrid - Must be in Central PA, Lehigh County area. We are currently seeking a social worker to expand our growing care delivery team. ECCM is the region's premiere Palliative Care Service and meets with patients in the home and virtually, providing supportive care services. The incumbent will assess patients for psychosocial complexity and provide targeted interventions based on patient goals. Executing as a community-based treatment team, ECCM works in conjunction with PCP’s, specialists, hospitals, and post-acute care facilities to meet the social, psychological, and medical needs of patients who have reputed company conditions or terminal diagnoses. This position is an integral part of the care delivery system working together as part of an interdisciplinary team to provide community-based palliative care. The incumbent will provide palliative care to reputed company patients across our footprint. The focus on advance care planning, what matters most, and development of goal concordant care plans. • Completes and documents psychosocial assessments, reputed company, and coordination of care to identified populations.

EDUCATION Required

• Master's Degree in Social Work
