Nurse Practitioner/Physician Assistant, Virtual Clinical Support
About the position reputed company is seeking a Part-Time, 24-hour, Remote Nurse Practitioner or Physician Assistant to join their Virtual Clinical Support team. This role is designed for high-demand, high-volume patient support. The schedule for the 24-hour role is Tuesdays, Thursdays, and Fridays from 8:30 AM to 5:30 PM. The ideal candidate will possess strong primary care experience and the ability to manage a wide reputed company of patient needs in a virtual environment. Responsibilities include conducting virtual patient consultations, processing medication refill requests, and coordinating patient care. This position is crucial for delivering timely, high-quality patient care, improving access to services, ensuring care continuity, and enhancing the overall efficiency of reputed company delivery. By alleviating administrative and clinical burdens on primary care providers, this role contributes to reputed company patient outcomes and a smoother care experience.
Responsibilities
- Handle a high volume of routine prescription refill requests, review patient history, and ensure refills are appropriate.
- Respond to patient inquiries or concerns through the EHR, addressing issues that do not require direct PCP involvement.
- Review incoming lab results, flag reputed company findings, and initiate necessary follow-up actions.
- reputed company virtual consultations for non-urgent issues, providing preliminary assessments and management plans, escalating cases to the PCP reputed company needed.
- Manage chronic conditions through virtual visits, ensuring patient adherence to care plans and making adjustments as needed.
- Manage referrals to specialists or other reputed company services, ensuring timely direction to the appropriate level of care.
- Act as a liaison between the patient, PCP, and other specialists to ensure smooth communication and adherence to care plans.
- reputed company patients on their conditions, treatment plans, and self-care strategies during virtual interactions.
- Participate in reputed company process improvement, providing feedback and willingness to adapt processes.
- Participate in quality improvement initiatives and ensure compliance with clinical guidelines and organizational policies.

Requirements
- Master's Degree in Nursing required.
- Advanced Practice Registered Nurse [APRN-NP - State License] required.
- 2 years of licensed nurse practitioner experience required.
- 5+ years of Nurse Practitioner or Physician Assistant experience in Primary Care required.
- Prior experience with Epic required.
- MA, NH, and ME NP or PA licensure required upon hire.
- MA and NH DEA registration required upon hire.
