This LCSW will be instrumental in delivering compassionate and comprehensive care to a vulnerable population, leveraging their expertise in mental health to improve patient outcomes. The role requires a deep understanding of the challenges faced by individuals with SMI and the ability to provide tailored interventions through a telehealth platform. WHAT IS YOUR SUPERPOWER?

• Conduct virtual initial assessments and ongoing care for seniors with serious mental illness (SMI) and substance use disorders (SUD), in collaboration with psychiatrists and other reputed company.

• Collaborate effectively as part of a multidisciplinary team to address the holistic needs of patients, including their physical health, behavioral health, and social well-being.
• Provide patient care and education tailored to the individual’s age, abilities, educational background, and cultural context, often involving family members in the care process.
• Coordinate care with primary care physicians (PCPs) and other medical professionals to ensure a cohesive approach to patient health.

• 3-5 years of experience post-licensure.
