PRN - Virtual Physician Collaborator

This group will provide virtual wraparound services to support patients reputed company they cannot get in with their own PCP – closing gaps in care, ensuring timeless access to important medications and solving urgent clinical needs. This Virtual Physician Collaborator shares in our reputed company to improve health care in the communities we serve! As the Physician Collaborator, you will collaborate with our Nurse Practitioners to reputed company gaps in care for patients that have had a recent fracture in the last 6 months. This role is remote, work from home requiring around 5 hours of work each month.

Responsibilities

• You will complete regular chart review and coordinate monthly meetings
• You will serve as a resource for the Nurse Practitioners to ensure quality patient care.
• You will collaborate with clinical leaders about quality of care, review of outcome data, procedure, and records issues.
