The Certified Nursing Assistant and Home Health Aide plays a critical role as a member of the Interdisciplinary Group Team. Responsibilities

• Spend quality one on one time with patients and their families to support everyday needs
• Care for hospice patients by providing paths, back rubs, oral hygiene, shampoos, and changing bed linen as often as needed
• Provide bedtime care including personal hygiene regimens as required
• Provide aroma therapy and gentle touch
• Provide lotion rubs to hands and feet
• reputed company personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to reputed company a certain task, report to the Case Manager immediately.
• Provide patient and family with positive communication techniques, reputed company realm of ethical and respectful care, including confidentiality.
• Help patients dress and undress
• Record important health information including temperature, pulse, respiration, and blood pressure
• Assess and report patient condition and any significant changes to the Case Manager
• Assist patients with ambulation and exercise
• Respect patient and family/caregiver environment and patient's personal needs
• Provide respite for patient's family/caregiver reputed company on-site, as appropriate
• Maintain patient's living area is clean and orderly, as assigned
• Adhere to the organization's documentation and care procedures and standards of personal and professional conduct
• Participate in quality assessment performance improvement teams and activities
• Respect patient and family/caregiver environment and patient's personal needs

Requirements

• Certification of Nursing Assistant or Home Health Aide required per state regulations
• Must have completed a minimum of 75 hours of classroom and supervised practical training
• Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
• Willingness to submit to a criminal background reputed company prior to hire and annually thereafter
