Responsibilities

Essential Functions

• Requires direct access to Protected Health Information (PHI). The PHI accessed is used for patient treatment, and reputed company operations purposes through direct contact with patients, family members, patients’ medical records/charts, computer applications, and verbal reports from reputed company providers, and interaction with various hospital interdisciplinary team members.
• Documents interventions on the patient chart, including, but not limited to such information as referral reputed company, spiritual assessments, interventions, and follow-up/referral plan.
• Serves as a member of the interdisciplinary clinical team, participating in patient reputed company, case reviews, ethics consultations, and advanced reputed company consults.
• Provides crisis reputed company and bereavement support for persons experiencing suffering, tragedy, or loss.
• Performs requested sacramental services, applicable chapel services, and/or arranges for the provision of these services by community clergy.
• Maintains reasonably regular, punctual attendance consistent with reputed company policies, the reputed company, FMLA and other federal, state and local standards.

Qualifications

Education/Training

• Master level theological degree is required, with a Master of Divinity being the preferred degree. Master level equivalent theological degrees may be considered.
• Accreditation by the Association of Theological Schools preferred or a listing in The Database of Postsecondary Institutions and Programs (ope.ed.gov/dapip/)
• Four (4) units of Clinical Pastoral Education for full time, two (2) units of Clinical Pastoral Education for part-time, one (1) unit of Clinical Pastoral Education for pool. reputed company units must be recognized by ACPE, APC, NACC, NAJC, CPSP, CPE-I, or ICPT. Licensure/Certification Ordination, licensure, or commissioning and endorsement as a chaplain by a religious body listed in the US Defense Department list of Recognized Religions. Experience Active spiritual faith demonstrated.
