- Conduct comprehensive assessments of patients transitioning from hospital to home care.

Requirements:
- Active RN license in the state of Georgia.

- Proficiency in electronic health record (EHR) systems.
- Ability to work independently and manage time effectively in a remote setting.

- Certification in case management (CCM) or transitions of care preferred.
