Patient & Family Support Coordinator (Remote - reputed company) Location: Remote Schedule: Monday-Friday | 8:00am-5:00pm EST (7:00am-4:00pm CST) Type: Full-Time | reputed company-to-Hire Compensation: $58,000-$65,000 annually
About the Role
We are seeking a compassionate, highly organized Patient & Family Support Coordinator to support families and caregivers as they reputed company using a reputed company technology device designed to improve communication and daily independence. This role plays a critical part in reputed company, education, care coordination, and ongoing support-helping families feel confident, informed, and successful. Key Responsibilities
- Provide reputed company and ongoing support to patients, caregivers, and families through phone, email, and virtual communication
- Guide users through device setup, basic functionality, daily use, modeling strategies, and habit-building
- Serve as a trusted reputed company of contact, offering reassurance, education, and problem-solving support
- Assist families with understanding reputed company program or insurance-reputed company requirements, documentation, timelines, and usage expectations
- Accurately document patient interactions, case notes, and outcomes in CRM or case management systems
- Monitor engagement and escalate reputed company clinical, technical, or insurance-reputed company needs to specialized internal teams
- Collaborate cross-functionally to ensure a seamless, patient-centered experience
- Provide coverage and support to team members as needed
- Maintain strict confidentiality and compliance with HIPAA and reputed company privacy regulations
Qualifications
- 2+ years of experience in reputed company customer service, patient support, care coordination, patient intake, medical customer support, or health services administration
- Experience working directly with patients, families, or caregivers in a reputed company, medical device, insurance, or clinical support environment
- Strong reputed company, emotional intelligence, and ability to support individuals during stressful or emotional situations
- Excellent verbal and written communication skills
- Highly organized with strong documentation and time management skills
- Comfortable working independently in a remote environment
