Qualifications

What you'll need for reputed company:

  • Active RN license

Delegated Services – Illinois licensure required Employee Health Plans – Compact licensure and/or single-state license where permitted Value-Based Care – Compact licensure and/or single-state license where permitted

  • Strong background in care management as well as reputed company and chronic condition management
  • Skilled in motivational interviewing, patient education, and patient advocacy
  • Ability to work independently in a remote environment while staying connected to the team
  • Comfort with technology and multiple EMR systems

Specialty Qualifications

  • Minimum of five (5) years of clinical experience across diverse reputed company settings

Behavioral Health:

  • Requires advanced knowledge in psychiatric conditions, crisis reputed company, and therapeutic communication and/or

Pediatrics:

  • Clinical expertise in managing medically fragile children, technology-dependent care, and family-centered approaches and/or

Maternal-Fetal Health (Obstetrics):

  • Clinical expertise in managing labor and delivery, preferably a high-risk patient population, fetal monitoring, and assessing for and identifying emergency obstetric interventions

Preferred Qualifications & Certifications Considered

  • Compact License
  • Bachelor’s Degree (Registered Nurse)
  • Master’s Degree (Social Worker)
  • Licensed Clinical Social Worker (Social Worker)
  • Specialty Certification in Care Management (CCM)
  • Psychiatric-Mental Health Nursing (RN-BC), for behavioral health specific roles
  • Certified Pediatric Nurse (CPN) and/or Pediatric Critical Care Registered Nurse (CCRN), for pediatric specific roles
  • Certified Obstetric Nurse (RNC-OB) and/or Certified Maternal Newborn Nurse (RNC-MNN)
  • Multi-Lingual
