Weekend Home Health Intake Coordinator
Reliant at Home is seeking a Full-Time Remote Home Health Weekend Intake Coordinator! The hours of this position will be 9AM - 6PM, Thursday - Monday! Job Qualifications: Education: High School Graduate Experience: 2-3 years Home Health intake experience preferred, LVN candidates accepted Skills: Required computer proficiency and excellent interpersonal, organizational, and phone skills. Must be able to move at a fast pace. Functions: Receives and processes new referrals in a timely manner. Facilitates care coordination with sales reps, referral sources, physician offices, patients, and agency clinical leaders. Effectively communicates and relays accurate care coordination details to internal and external constituents in the referral coordination process. Takes ownership of recording accurate information on the referral tracker and in the EHR and ensuring all referrals move through the process quickly and responds and communicates in a professional manner.
