How you’ll make an impact in this role Lead the coordination of specialized nursing programs to ensure patients receive seamless, high-quality care within our clinical practice. Create and advocate for clinical standards by serving as a primary resource for nursing staff, ensuring our team operates at the highest level of excellence and safety. Design and implement personalized care plans that translate therapy goals into actionable steps, directly improving patient recovery outcomes. Optimize clinical resources and equipment to ensure the care team has exactly what they need to deliver uninterrupted, top-tier medical service. Manage vital care documentation to safeguard the patient journey, ensuring every record is accurate, compliant, and reflective of the care provided.

Responsibilities

  • Lead the coordination of specialized nursing programs to ensure patients receive seamless, high-quality care within our clinical practice.

Requirements

  • Practical/Vocational Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
  • Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Nice-to-haves

  • Execute high-touch patient navigation by leveraging telephonic outreach and coaching to close HEDIS quality gaps, ensuring patients remain engaged and compliant with their care journeys.
  • Optimize clinical work-flows and documentation through expert-level chart preparation and EHR management (Athena, Innovaccer, or Cerner) to support seamless collaboration between staff and clinicians.
