Chronic Care Management-Health Coach - (Day shift)

About the position Cardiovascular Services of America (CVAUSA) is the largest private and independent cardiology network in the United States. ESSENTIAL DUTIES AND RESPONSIBILITIES (included but not limited to): We are looking for candidates who want to use their expertise to improve health, wellness and care access for patients with chronic conditions. We are recruiting candidates that have experience with cardiology patients, able to develop purposeful relationships with patients, and can provide clear reporting to the cardiology care team. The CCM assistant has the following responsibilities: Develops a patient coaching relationship that supports the patient care plan and quality of life for the patient It is a trusted and empathic support for patients and families dealing with chronic conditions Communicating abnormal findings to the cardiology care team. Monitors and encourages patient adherence to care plans and participation Effectively communicates findings to the care team Supports new patients in the enrollment process Works as a liaison for the patient with the practice and providers Identifies and escalate changes to providers to reduce events such as hospitalizations or urgent care visits Works effectively with the EMR and CCM platform Basic monitor troubleshooting with patients for the CCM platform Ability to follow clinical protocols. Participate in continued team education through shared learnings. Contribute to a high-quality service both to physicians and patients. Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.

Requirements

  • High School diploma or GED
  • 1 year of experience in remote chronic care management
  • Experience with cardiology patients - specifically heart failure
  • Registered Nurse (RN)
  • Medical Assistant (MA)
  • Certified Nurse Assistant (CNA)
  • Experience with EMRs – specifically Athena, NexGen, GEMMS and e-clinical works
  • Great communication and multi-tasking skills
